Requirements and Guidelines
This program serves two distinct cohorts. Please see below for specific eligibility requirements for each.
Cohort A: Moms Ages 18-22
Reside in Arkansas,
Be 18 to 22 years of age,
Be currently 27 weeks pregnant or less,
Have an annual household income of $44,000 or less annually.
Cohort B: Moms in the Armed Forces
Reside in Arkansas
Be 18 years of age or older,
Be currently serving or have previously served in the U.S. Armed Forces, including the National Guard and Reserves
Be currently 27 weeks pregnant or less
Have an annual household income under $44,000, excluding VA disability compensation and veterans' benefits.
No SSN or ITIN required.

21230 — Cherry Hill
21215 — Park Heights
21740 — Hagerstown
Be at least 18 years of age
Be 27 weeks pregnant or less
Be eligible for Medicaid
No SSN or ITIN required
Requirements and Guidelines
All applicants must:
Live in one of the ARC-designated counties within the states of Kentucky, Ohio, West Virginia, or Tennessee
Be at least 18 years of age
Be at most 14 weeks pregnant
Submit proof of pregnancy that includes:
Patient's name
Clinic's name, location, and contact info
Date of visit to medical provider
Expected due date
